Community Health
The Community Health Department exists to address and reduce barriers to health that impact our patients and the broader community. We are committed to extending care beyond the walls of the clinic by providing access, encouragement, education, advocacy, and connection to resources that promote healthy and resilient lives.
Community Health services are available to all individuals in the community—whether or not they are established patients of Exalta Health—reflecting our mission to serve with compassion, dignity, and Christ-centered love.
The work of the Community Health Department is organized into two primary focus areas:
Community → Exalta Health
This focus area emphasizes:
- Building relationships with community partners and organizations
- Engaging community members through outreach, education, and collaboration
- Coordinating volunteers and fostering community involvement
- Ensuring pathways for individuals and organizations to connect with Exalta Health services
Exalta Health → Community
This focus area emphasizes:
- Supporting patients beyond clinical care through navigation and follow-up
- Addressing Social Determinants of Health (SDOH)
- Connecting individuals to community-based resources and supports
- Advocating for patient needs and promoting long-term wellness
Our Commitment
The Community Health Department of Exalta Health is dedicated to walking alongside individuals and communities with compassion, excellence, and faith. Through collaboration, advocacy, education, and service, we strive to break down barriers to health and empower individuals to live healthier, more supported lives.
