Our goal is to provide an accessible Path to Health that improves community well-being through compassionate, culturally responsive, and transformative in-home visits. By collaborating with families and community partners, we strive to equip individuals with the knowledge and resources they need for a healthier life—one household at a time.

Are you eligible for services?

To qualify for this program, you must:

  • Be over the age of 18
  • Be a resident of Grand Rapids, Wyoming, or Kentwood
  • Not have a primary care provider
  • Not be pregnant
  • Not be a current patient of Exalta Health
  • Have limited access to primary care

Navigating Insurance

Having health insurance is not necessary to be a patient at Exalta Health. In fact, we specialize in serving those who do not have insurance. However, many people who are eligible for affordable insurance do not have it. We can help you sort out if you are eligible and how to sign up if you are.

Medical Cost Resources

Prescription medication can be the most expensive part of health care. We can help you find the most affordable prescription medications. Financial assistance directly from the drug companies may be available even if you don’t have prescription drug coverage as part of an insurance plan.

Fátima Vega-Colón

Fátima Vega-Colón is a graduate from Michigan State University where they graduated with a Bachelor’s of Science in Anthropology. Their passion and career focus is to facilitate services to those who are minoritized and underserved. They are one of the Community Health Workers (CHW) for the Path to Health program where they connect individuals to essential care through in-home visits. Through this role they hope to empower individuals with knowledge and resources to achieve healthier lives, aligning with the program’s mission of transformative and accessible health care for the community.


Fátima Vega-Colón se graduó de la Universidad Estatal de Michigan, donde se graduó con un Bachillerato de Ciencias en Antropología. Su pasión y enfoque profesional es facilitar servicios a quienes están minorizados y desatendidos. Es uno de los trabajadores de salud comunitarios del programa Camino a la Salud, donde conecta a las personas con cuidado de salud esencial a través de visitas a la casa. A través de este rol, espera empoderar a las personas con educación y recursos para lograr una vida más saludable, alineándose con la misión del programa de brindar atención médica transformadora y accesible para la comunidad.

Henry

Henry was born in Grand Rapids and earned his Bachelor of Science in Nursing (BSN) and
Doctor of Nursing Practice (DNP) from Grand Valley State University. Passionate about serving the Hispanic community in which he was raised, Henry is dedicated to providing compassionate care to minoritized and underserved populations. As a Nurse Practitioner for the Path to Health program, he focuses on delivering essential care through in-home visits, helping individuals access the healthcare they need in the comfort of their own homes.
