Medical Health
Diabetes Clinic
Diabetes is the most frequent diagnosis at Exalta Health. It is a complex, chronic, and progressive disease that affects many physical systems. Learning to manage the disease is a real challenge.
We have a special Diabetes Clinic with four key components:
- Education is provided in both English and Spanish. Patients learn more about diabetes, how to make good diet choices and develop self-management tools to help control sugar levels.
- Dental screening is provided to detect progression of diabetes through oral diseases.
- Vision screening is provided to detect retina disease or complications. Some diabetic complications such as retinopathy do not have obvious symptoms, so eye screening is critical.
- Examination – Patients review their medications with the medical team, receive a foot exam, have blood drawn for labs and discuss their diabetes treatment.
Dental Health
We are not accepting new patients at this time.
Dental experts provide compassionate oral care, which is critical for overall health, self-esteem, and success.
Our dental services include:

Dental Health
Senior Dental Program
Exalta Health provides dental services to eligible seniors, thanks to support from Area Agency on Aging of Western Michigan through the Senior Millage program. Low income seniors who do not have dental insurance coverage may qualify for the service at no or reduced charges. Services include general dental services, dentures and partials, and root canal treatment.
Mental Health
Physical and mental wellness are deeply interconnected, and we provide resources for emotional and spiritual health. Our behavioral health services include treatment for depression and anxiety and substance-use concerns.
Spiritual Health
We believe that we are created as whole beings – mind, body and spirit. Often when our body hurts, so does our spirit. At Exalta Health, our chaplain and staff are available to listen, provide comfort, and pray.
Community Health
Our vision is to provide an accessible Community Health program that improves well-being through compassionate, culturally responsive, and transformative support. At Exalta Health, we know that staying healthy means addressing the social and economic barriers families face, which is why our Community Health Workers build trusted relationships, offer personalized guidance, and connect patients to vital resources. By collaborating with households and community partners, this program strengthens our care model and empowers individuals to take meaningful steps toward healthier and more resilient lives, one home at a time.
